Platelet-Rich Plasma (PRP) is a hot topic orthopedics and regenerative medicine. It involves extracting a patient's blood, processing it to concentrate platelets and growth factors, and injecting it into the target area to enhance tissue repair and reduce inflammation.

The key premise of PRP therapy is that platelets promote healing when injected into damaged tissues, potentially aiding recovery from conditions like tendon injuries and osteoarthritis. PRP is viewed as an innovative alternative for individuals seeking pain relief and improved function.

All this promise and excitement begs the question: Does PRP actually work?

Overview of Current Literature on PRP

PRP therapy has emerged as a promising treatment in orthopedics and sports medicine. Studies have shown PRP to outperform steroid injections in treating many conditions, with superior outcomes in pain relief and function.

Research in Archives of Physical Medicine and Rehabilitation indicates PRP improves knee osteoarthritis symptoms compared to conventional therapies, enhancing patient outcomes across musculoskeletal disorders.

Effectiveness of PRP in Osteoarthritis Treatment

The American Academy of Orthopedic Surgeons stated that scientific evidence supports PRP for knee arthritis treatment over steroid injections, hyaluronic acid, NSAIDs, exercise, ozone, physical therapy, and bone marrow stem cells. JAAOS

Similarly, The American Medical Society for Sports Medicine (AMSSM) concluded that PRP injections are superior to steroid or hyaluronic acid injections for knee osteoarthritis, particularly in younger patients with mild to moderate disease. AMSSM

PRP for Rotator Cuff Injuries.

PRP therapy has shown efficacy in enhancing rotator cuff repair outcomes. A recent meta-analysis from the American Journal of Sports Medicine found that rotator cuff repairs enhanced with PRP have fewer repeat tears than in patients who received a standard repair.

Another recent double-blind trial showed that in patients with rotator cuff tendinopathy who had failed other treatments PRP outperformed steroid injections for functional and pain improvement even at 12 month follow up.

Compelling Evidence for PRP: Lateral Epicondylitis and Beyond

PRP has emerged as a treatment for lateral epicondylitis with trials showing significant pain reduction and potential benefits in arthritis progression, prompting interest in PRP as a primary option for soft tissue injuries.

Emerging Data on Cartilage Regeneration with PRP

In 2022, a large, multi-center randomized controlled trial compared three high-dose PRP injections (administered one week apart) to placebo injections. Compared to placebo, the PRP group experienced nearly 50% less arthritis progression as measured by cartilage volume on MRI done 5 years later!

Emerging data also suggest that PRP could promote cartilage regeneration. In this 2024 study, patients who received three PRP injections one month apart showed improvement in knee cartilage thickness from an average of 0.92 mm at baseline to 1.01 mm at six months.

The Bottom Line on PRP Effectiveness

The evidence supporting PRP for many orthopedic conditions has strengthened dramatically in recent years. Several major orthopedic societies now endorse it, and the research now consistently shows it outperforms a number of other therapies.
